Planet Hollywood Meal Voucher Question

Can someone please explain this to me? What are the meal choices and do you get desset, drink (non alcohol of course) and appetizer with it? I mostly need to know the meal choices.

I just got that for my trip last week and didn't use it. I think it was only for an entree, and it was a limited selection- choice of burger/sandwich or salad basically. The merchandise voucher was for a tiny little keychain, if you have that too.
