Photo of reciept when using dining plan?

Does anyone have a reciept they can post a photo of that shows how the reciepts spell out for you what a 15/18/20% tip is, and you circle which one you want? (Do they still do this? they did in 2010)

Due to some trip adjusting due to flights, the first meal DD and DFriend eat will be without DH and I, and I wanted to make sure DD knows how to handle paying (she'll have charging privs on her KTTW JUST for tips because she and her friend will eat 3-4 meals without us.)

I just want to be able to show her an example so she's not confused and the server doesn't have to take the time to explain it to her.

don't have a photo of receipt but we were just there last weekend on the regular dining plan. They print the 18% and 20% suggested amounts (there is no 15% suggested amount)

...you have to write the amount in on the tip line, and total it. And the servers are actually quite nice about explaining which receipts go to you, and which are to be signed and return, as well as show you where to put the tip amount in

Thanks! I just feel bad about dumping her off at a restaurant - the original plan was to arrive in time for dinner the night before, and then when we finally found flights, we had to cancel that dinner reservation. I'm sure the girls will be fine, just a little bit of mother's nerves.
