passports design for girl scouts

My daughters girl scout troop is working on their travel badge and I'm helping do the class. I am looking for a passport design to have the girls do if one of you great designers can do me one with a cover design too.

There is a DISigner on here that has a GS cookie mouse head. The body has GS cookies and in one ear it says Cookies got me to Disney. An idea in case you want t-shirts
