People get congratulated for reaching goals by their friends. Whether that goal is losing weight, graduating college, finishing a marathon, getting married, or whatever you congratulate them because as their friend you are showing support. I think people congratulate strangers on accomplishments partly out of politeness, sure, but what is wrong with being polite.
Of course it doesn't hurt if whomever the person is looks better now because they lost weight.

Who cares how the weight came off? Anyone who knows anything about weight loss surgery knows that it's NOT the "easy way out." I wish my sister's doctor would be as assertive as yours. She's over 350-lbs and has some serious health issues. At this point, she is just resigned to it. She could lose 100-lbs and she'd *still* be morbidly obese so she just says the heck with it.
