Onpoint clarification?

Sorry if this is a duplicate already discussed but I couldn't find reference.

I paid for my 2 telehealth appts on OnPoint.
But the email domain @onpoint-testing.com, (prefaced by a persons name) it asks me to complete patient registration via link cov19.health ....this seems different than the links that I saw in DIS threads?

Also, it wants the drivers license photo?

Finally, where it asks for insurance info, it doesn't give option for self-pay, but there is option for uninsured. Shld I select that? We have insurance, but already paid for the telehealth with cc.

I unfortunately did not take screen shots, but I was directed to the Cov19health website to register. I did not have to submit my license, but they instruct to have government ID available at testing. I don’t recall the exact wording of the 3 options, but one was insured, uninsured and self bill or something like that. I just checked the option that wasn’t insurance related and finished up.
 
The email I received that contained the Zoom link also contained the instructions to register at cov19.health. We were not asked to provide photos of our driver's licenses, and we chose the "self pay" option under the insurance section. That was a few weeks ago, maybe the website has changed.

Another question...if 2 of us are going to be on the same telehealth appt, when we register on cov19/HealthTrack, can we use the same cell number and email address or do we have to use separate email addresses and diff cell numbers?

I used the same email and phone for everyone since manage it all, and I didn't have any problems and I received an email and text confirmation for each person.
 
Where did you all find PCR or NAAT tests Disney requires? I have the tests from the government but they are not PCR or NAAT. We have our OnPoint scheduled.
 
I answered on the other thread:
If everyone is vaccinated, you don’t need PCR/NAAT, antigen will do as long as it’s proctored. If you have an unvaccinated little one, you can order a PCR test from Inspire that you mail back (not online proctored).
