Just wanted to share an odd experience, in case anyone else had the same and is here searching for an answer. 
I ordered 4 Premium Annual DVC Exchange vouchers for an upcoming trip and tried two ways to link them to my MDE/MDX account: entering the number manually (on phone and on computer), and scanning the bar code using my phone. In both cases I received messages that the pass was ALREADY LINKED TO AN EXISTING ACCOUNT.
I called Disney Tech Support (407-939-7765) and they were able to resolve the issue -- I now see them in MDE and it looks like we're all set. The CM was VERY HELPFUL and the call was pretty painless.
The CM I spoke with was puzzled at first and had to IM a colleague in another department who has access to the system that links vouchers to accounts. That second CM found that while it was returning an "already linked" message, it didn't bring up any particular account that it was linked to! He was able to link to my account and we're all set.
Would certainly have preferred not to encounter this oddity, but really painless to fix.
