IMO, strategies for FP+ depend not just on what day you're there (crowd levels), but what park you're in. (FTR, we went in November 2014 over the Wine and Dine 1/2 Marathon weekend).
MK in particular we found we approached differently than the other parks, just because of the sheer number of attractions, and how the crowds disperse. With Anna and Elsa and 7DMT, we found Adventureland and Frontierland to be empty until appx 10:45/11am-ish. We rode BTMRR a few times back to back with only maybe 5 min waits in between (time to get off, walk around and through the queue again and maybe 2-3 mins in the queue), POTC was walk on, Jungle Cruise was walk on..we found CHip and Dale coming out for a M&G, no line at all..HM was about a 15 min wait around 10:30 or so. We did not go to Tomorrowland first, but I heard reports that Tomorrowland was similarly empty at RD.
All that's to say, for MK I would probably do the lands like AL, Frontierland, and TL first thing in the morning (since you're trip is a long trip, it seems as though you could have enough days to do each land at RD one day of your visit?) - headliners AND secondary attractions - and then use FP+ in Fantasyland or for any headliners you want to wait to ride in the afternoon (like Splash).
For Epcot I would FP+ either Soarin or TT and do the other headliner at RD (and, again if you have enough epcot days, maybe schedule 1 Illuminations FP+ one day?), and then work on secondary attractions.
DHS - if your party wants to ride both TSMM and RNRC, I would FP+ one and ride the other at RD, then move on to secondary attractions. DHS I feel like this is a little easier to do because there are so many shows that you can do in the mid afternoon- like Lights, Motor, Action, Indy, BatB, VotLM...where capacity is much higher. I would spend the morning doing "ride" attractions, and the afternoon doing shows. (Similar to what you wrote

AK - I would probably take an approach similar to DHS - rides in the morning, shows in the afternoon. You could easily get multiple rides on headliners here by arriving at RD and doing rides SB while holding a FP+ for later morning/early afternoon.
Personally, we did not find much success with getting more than our prebooked 3 FP+ on our November 2014 trip. I think a large portion of that was because we scheduled our FP+ in such a way was they weren't done until at least 3pm, sometimes later, and there was just nothing left - both at MK and DHS (the only places we tried). If I were trying to consistently get a 4th FP+ and beyond, then I would personally schedule my FP+ to be used up by 1pm. The shorter hours across the board in November, coupled with the party hours, just really seemed to affect additional FP+ availability.
