Need Help With WDW Passholder Website

For some reason I cannot figure out how to put in my info to use the passholder portion of the wdw website. My desktop has it saved, but now I'm using my laptop for the first time it is asking for me to put in my pass info. I'm having trouble with the last box. I have only 2 numbers to put in. I also don't know if I should be putting in the ticket purchase date or the exp date, I've tried it both ways but it's not saying error or anything, just taking me back to "add your pass"

Can anyone help?
