Need help to find something

Last trip DH and I took in 2008 I used a templet to make a journal of sorts. It had:

Which park visited
Weather
which rides I liked best
Where we ate
Etc.

Can't find the site again to make more. We leave on Thurs and would like to prrint some off before we go. It really help with scrapbooking the trip.
