Need help finding simple Journal pages

When DH and I went to disney in 2008 I got some journal pages off of this board. I would love to make some for our trip leaving on Thursday.
The pages said:

Where I went
What Park
How was the weather
What rides did I like best

Etc.

Does anyone remember them or have a disign that I can print out.
