Need document allowing grandparents to have Medical authority

Hi All.

I surprised my wife with a quick trip to Disney to celebrate our fifth anniversary and to give her a little break as she has been taking care of our first born.

This is our first trip without our baby as she will be staying with Grandma & Grandpa.

We want them to be able to have the authority to bring our child to the hospital (God Forbid) if need be.

Does anyone know of the name of the form that I am looking for and where I can find one?

Just did this in September. Basically, we typed a letter with all the appropriate info, including social sec. #'s, insurance #'s, we signed it and had it notarized. We decided to list another individual, besides the couple babysitting, just in case the hospital/doctor etc. couldn't reach the first couple. We also made a copy of the insurance card.
 
If you got to Medids.com, there is a form that can be printed out. It is the one I always give to the grandparents.

We needed this type of stuff one time too. I called my DD's clinic, and they have a standard form for allowing my parents to seek care at the clinic for my DD. It had all the info the clinic would need if my DD needed to be taken to the clinic. We tried to get a similar form for the hospital, and they said the clinic form was adequate.
