My Adventure Book help please

I am looking to make my kids an adventure book to help tell them they are going to Disney and that they can also use for signatures while there! I am wanting a cover that looks like the adventure book from UP that I can print. Preferably instead of saying "Our Adventure Book" like in the movie, I was hoping for it to have each child's name on it to read: Kate's Adventure Book, Tristan's Adventure Book, Madalyn's Adventure Book. Otherwise, I would like it to say My Adventure Book. Please and thank you!
