Is this fairly easy? I have OBB benefits on a cruise but would like to book one of the early 2019 cruises when they open. How do I make sure the OBB is on there?

Either way when you book the new cruise it is associated with the OBB so it is automatically there. I booked a placeholder onboard with Costco as the TA. Once Summer 2018 itineraries were bookable for Gold, I called them and they used my placeholders to book my staterooms.
You want to change the cruise you currently have booked with Costco to a new cruise? You will have to go through Costco - they need to change it for you. If the currently booked cruise has the OBB, the benefits will automatically transfer as long as the new cruise date is within the 24-month period. I don't know if Costco will take that request in advance and then do it for you on the correct opening day, or if you will need to call Costco that morning.

I had to fill out the reservation transfer paperwork so now I don't trust what should be an "automated" system.

That's how it worked for me when I booked Alaska in February using a placeholder I booked August 2016.I'm curious, did your email start with, "Dear Travel Partner" or "Dear (your name)"? I also have a placeholder assigned to Costco, but was under the impression that when I was ready to book I'd just provide them the reservation number and they'd apply it to whatever cruise I wanted. No paperwork required. I received the email addressed "Dear Travel Partner".
