Minors passports

My son is 4, his father and I have joint custody but I am the residential parent, do I need his father to fill out the info, and do we need to supply the custody paperwork with the passport, any info is greatly appreciated, his father will get the needed papers notarized, but will not go with us to the passport office, also do I need to get a letter notarized from him when we go on our cruise stating that he gives his permission. Any info appreciated. I need to get both our passports soon.
 
DH: & I live together and are happily married, however, he couldn't come with DS13 and I when we needed to get DS's passport renewed. He just wrote a letter stating that he was the natural father and gave his permission for the passport to be issued, had it notarized and I gave it to the clerk. No problem. It can get sticky now that they are requiring both parents, though it is understandable. If you read the form, though, it says a letter, notarized will suffice.
 
I believe that a form comes in with the cruise docs that hubby can sign and get notarized.

Many people will tell you that they are in this same situation and either did not bring documentation from the other parent or had it and weren't asked for it. However, you never know when you will be asked for it. They just want to make sure that both parents know that the child is leaving the country and have given permission.
