medical consent form for grandchild?

We have a grandchild vacationing with grandparents at Disney. We were wondering what kind of medical release we need in case of emergency. Can a medical release be a hand written letter from parents stating the grandparents can give medical consent or do we need a formal form? Thanks!
 
I'm no lawyer or anything, but whenever we took kids on vacation with us growing up, my mom always had the parents write a letter saying that it was okay for my parents to get medical attention if the need arises. I also watch my friend's daughters during the day, and I just googled medical release form and day care and a nice one came up. (I'm not sure if that was the specific search, but something like that). It just basically says that if we can't get in touch with the girls' parents, then I am authorized on their behalf to get the ball rolling.

It's a good thing to have though! Good thinking!
 
When dd vacations with other families we just type up a paragraph giving consent for the other parent to make decisions in the event we can't be contacted and have it notarized. I will put the dates of the trip on the letter and send a copy of her medical insurance card. Grandparents are nice to take the kids on this trip! Hope all have fun!
 
My DD's pediatrician has a form that we use. We never had to have it notarized, just signed by both parents. Make sure that you also have the insurance card for the child so you don't have a red tape nightmare.

Same advice...my Ped has a form too...we do have it notarized just for good measure because what they require here may be different from what they have there!
 
The hospital that I work for requires a form to be notarized. The laws vary from place to place, and it's not hard to get something notarized. We couldn't accept anything that wasn't, because anyone could write anything down and sign the parents name, and we would have no way of knowing.

Don't take that the wrong way, I'm not saying you would do that, it was the reason behind the rules for us, I guess.
 
My parents traveled with DS 4 years ago. I typed up a form with his allergies, health history, DH and I's home, work and cell numbers our doctors office number as well. I had it notarized. It gave me some piece of mind.
