Medical Consent/Authorization for Kids?

DH and I are taking our nephews for their first trip to WDW. Does anyone know where I can find a generic form for medical consent and authorization?I've been searching online and they all seem to be specific to organizations or events. The older one has asthma - we are comfortable with this and don't anticipate any problems but would rather be prepared.

Thanks for any assistance, everyone has been so helpful on my other thread asking for tips since this is our first time going with kids!
 
I don't know of a generic form..if there is one I'd like it as well. However, if my children are spending a week with their grandparents or something I just write a generic statement

I ....give permission for ..... to have my children..... to be treated in the event of an emergency.

Fortunately they've never had to use it, but we have used such a written statement for my cousins one year when they stayed with us in the summer as kids. The hospital accepted that as permission.

you might want to have the parents call their insurance company-some have a blanket form that can be used (and it also clears the way for you to be able to authorize services that the insurance company will pay for).

you can always take an event type one and use it as an example to type up your own-just remember to get BOTH parents or legal guardian's to sign and get it notarized-even if both don't have physical custody. one of my co-workers got into a major issue after the fact when a relative child traveling with her got ill and the non custodial dad who carried the kid on his insurance got all the medical bills bounced plus pursued legal action because while the mom who had physcial custody signed a release for treatment-he did'nt:sad2:
 

Your Pedicatric office should have some forms. Just make sure you have it, and a copy of their insurance card!
 
a lot of hospitals have the forms on their websites too.
If you find a specific one you like, just copy it and cut out the things that are not appropriate.
 
just thought-while a general med auth might allow for treatment, hipaa laws might cause probs. re your authority/ability to allow for access for med providers re. existing med. records/treatment. might want to cross ref. hippa to ensure you have access in need.
 
Thanks so much for the replies. I hadn't thought of getting a copy of the insurance cards...that would be important if we need it!

Also good tip about having both parents sign...one of the nephews' parents are divorced.

Also, I always put all of mine, my husbands and my kids info on the form. Birthdays, SSN#, etc. Some insurance companies use that info as identifying information. I also have the form read "authorize treatment and/or emergancy care." That way they can take the kids to the doctor if they are just sick but not in an emergancy. Also, don't forget to Notorize and have effective dates. Here is what my last letter looked like:

Lorie Rose is authorized to approve emergency care and treatment of Zoey Brown (XXX-XX-XXXX) and/or Kasey Brown (XXX-XX-XXXX) in my absence. She has my permission to make all decisions concerning Zoey and Kasey.

I never even thought of this, what great advice I always find here, lol! My sister is taking my daughter to Arizona for a week on a friend's ranch next week, I'm printing this out right now for her to take. Thanks!
 
Make sure you have this notarized. I travel frequently for work and have to leave my DD with my parents. My pediatrician told me that many hospitals will not accept the form unless it has the notary stamp/embossing.

I was just going to say this. I'm pretty sure it was when my parents took my nephew to florida, they were told to make sure it was notarized!!

My parents took my son to FL and we had it notarized, as well as when we went away for a long weekend. Luckily, one of my employees is a notary.
 
For our tenth anniversary we left our girls with my dad and went on a cruise. Because we were traveling together for the first time without the kids we had our wills drawn up and at the same time we paid the lawyer an extra fifty dollars to draw up a temporary custody form. It had start and end dates as well as all the other relevent information. Since then I have used it as a template when we have needed to leave our girls with family. We update and notarize every time. We also make sure whoever is watching them has a copy of their birth certificates as well as the insurance cards. We also write up quick medical histories (allergies, etc.) and make sure that the sitter is familiar with our insurance (co-pays, approvals, etc.) It seems like a lot, but I would rather be over-prepared than have delays in treatment because some piece of information is missing.
 
When my parents took DS to WDW I typed up a form with his name, allergies, medical history, #s where I and DH could be reached as well as his pediatrician and his phone number. I signed it in the presence of a friend who is a natary. They didn't have to use it, but I felt better having it.
