ME reservation question

I thought I completely understood this ME thing!
Just booked our ME for round trip, online. There are six of us, one lodging reservation number, all same flights. ME asks for name of everyone in our party, but then says if there is more than one reservation, I need to complete another form. Does that mean LODGING reservation? I can assume that since we're all listed on one booking, that's all I need to do? Or do I have to do a booking for everyone? If just one, I can assume they'll provide enough tags for 6 people? THANKS!!
