License-Ariel2 with "Kristie"
License-Goofy1 ...one each for "Sam", "Erik", "Beth", "Joel"
License-Tinkerbell6 with "Addison's Stroller" or just "Addy's Stroller" if her full name is too long to look right
th_License-Minnie2 with "Kaitlin"
th_License-Minnie3 with "Addison"
th_License-Minnie5 with "Amber"

These are all for May 2012. Can you do "Disney Fantasy" instead of WDW? If not, the top can just be left blank.
