I hope you don't mind but due to your longish (and rather beautiful) surname, I decided to put just your dates on the front and name on the back. I tried to squeeze name and dates onto the front (as in the example) but it looked too squashed up Sorry!!

You don't say whether or not you are flying (it's the thing most people forget!!), so I left off the 'flight number' bit but I can easily add it back in if you wish.
