Lost tooth request certificate help!!

My daughter has a very loose tooth. We will be heading to Disney this thurs. if her tooth falls out I would love to have a cute certificate from tinkerbell and the tooth fairy that entitles her to a special treat.

Can anyone whip something like this up on short notice?

We told her that if she lost a tooth while we were in Disney that Tink would come take it and give it to the tooth fairy!
