Looking for MM head silhouette filled in with names

PLEASE HELP: Hi All! 2 years ago I had MM head silhouette's filled in with family member's names made for my family and I can't find the designer now. Help? Need:

Larry, Deena, Samuel, Madeline

Also, my husband is currently serving in Afghanistan, so any suggestions on how to "mark" he's not on this trip.
