Looking for An ADR planning page/journal page?

Since I planned the trip for everyone, I am busy making a little info book for everyone to have.

I have been desperately searching for a journal page that is disney themed (nothing too fancy)

that would allow me to write in the 8 different adr's, confim #, location, and whether we are paying out of pocket or on the dining plan.

does anyone have something that I would be allowed to use?

Look under the tab for Travel Pages, then WDW-Orlando. She did a generic dining planner page plus one for each park.
