kids immigrations forms

I'm trying to be good and have all my forms filled out before we leave. On the kids immigration forms, does the parent sign or do you leave it blank?? They really don't say, it just has a signature line. And also do you only need one charge authorization form or does that have to be filled out for everyone since you list those authorized to charge. Thanks

I sign it with his name followed by the words "by mother".

It does say on the folder flat part that parents may sign for minors...I took that to mean anything that required a signature for them.

Thanks, how about the charge authorization form, only 1 or do I have to fill out the ones in each of my childrens documents??

The instructions for that also say you only need to fill out one charging privileges form per stateroom. Check the flap on the envelope the docs come in....it basically tells you exactly what is needed in writing....says Helpful Tips on inside flap. If you have 2 staterooms than fill out the form for each adult who is main person for that stateroom.

Thanks, I may have misplaced that folder already!! I took the documents out to fill out and never looked at the flap!! Trying to pack and sort and get ready to go! Glorious last minute details!!
