Is my DH earning an appropriate salary?

A DO still needs 4 years of undergrad and 4 years of medical school.

Yeah, but it isn't an MD. A nurse practitioner also practices medicine without an MD...but in both cases a degree (and a graduate degree) is required - just not the MD.
 
These are very different than Civil Engineer. Companies want a PE to sign-off the job. I am sure either the owner or the brother is a PE. The OP's DH needs to complete his BS to even try for the PE license.

Just curious... does your DH get to keep his frequent flier miles & hotel points? You can also get an AMEX card that pays you for accumulated points (instead of turning them in for tickets / mdse). If the company lets you keep the points, then that is extra 'money' that you can use for family time/vacations, stuff.

Yes. He gets to keep all the Rapid Rewards credits and Priority Club points he accumulates. This has been a nice perk that I totally forgot about. We have taken over 15 free round trip flights with his RR credits and I couldn't tell you how many free hotel night stays with the PC points.

I have never meant to sound like we are ungrateful regarding DH's job because we are not. I merely wanted an idea of if he was in line with other jobs similar to his or not. Yes, we are going to ask for a raise in salary next year on DH's anniversary date due to the fact that it has been so long since he received one. Even a 2-3% cost of living would be great. I just wanted an idea of if we should be negotiating for me. Obviously not.

Also, I realize that bonuses are just that, bonuses. However, if you say that you would not be expecting one if you had a job such as my husband's, after having received one for several years with no exception...you are kidding yourself. If you had someone tell you "Good job!" at work after performing well, everytime you performed well, wouldn't you be suprised and a little let down if it all of a sudden stopped? Of course you would! Kind of the same situation. We had always received one, had been told upfront that bonuses were part of the job, never heard otherwise, and then all of a sudden..gone. I was just taken by suprise.
