If your loan had a missed payment due to an EFT glitch or even if you did not catch your credit card expiration soon enough you should certainly see whether a payment was missing from your bank statement or your credit card statement. Then you could match up any claim from a loan servicer or the IRS.
If the IRS took your refund to pay the debt of someone else perhaps with our same name, or sent your refund to some imposter who filed a return in your name then the IRS should eventually send you your refund and then worry about recovering the money from the incorrect person.
