We are going to Disneyland August 23 (!). I have only known for sure for about a month. It has been on again/off again. Anyway I just started planning.

On some rides (Pinocchios Daring Journey, Snow White's Scary Adventure for example) it says you need to transfer BUT then it says "Guests may request a wheelchair accessible vehicle." Does this mean that I can ride in my wheelchair? Or is it referring to the transfer accessible vehicle?

I don't know for sure and hopefully someone who has been to DL will answer and let you know for sure.
I saw a recent post where someone talked about a transfer accessible vehicle for Snow White at DL, so I'm pretty sure that is what they mean.
 
They're talking about a vehicle that is easier to transfer into not one where you may stayin your wheelchair and ride.

For example this is the Pinocchio vehicle, very simlar to the Snow White one. It has a little door on the side to make it easier to get into.

California Screamin has an interesting vehicle for transfers. One of the seats is steplike to lower yourself in one step at a time:

I remember seeing at Dw on Small world ride they have a boat where you can roll the wheelchair right onto it. not sure if thats what they mean or not. Interesting. Seeing I am bored I will see if I can find anything for you but you might be better to call and ask seeing time is short.
 
found this link it tells you for each ride at DL how you will have to tranfer or get onto the ride from a wheelchair. I hope this helps you out!

Scroll to the bottom of the list and you will see this explination :Transfer Access Vehicle – The attraction offers a unique vehicle to assist Guests with mobility disabilities in transferring from their wheelchair or ECV to the ride.Transfer Device – The attraction offers a portable bench or similar device to assist Guests with mobility disabilities in transferring from their wheelchair or ECV to the ride.
 
I only remember my mom being able to stay in her chair for Monsters, Inc, Winnie the Pooh and It's a Small World. I think Jungle Cruise is supposed to have a special boat, but it was out for refurb when we were there. The only 2 rides my mom rode that she transferred for were Pirates and Soarin'. She didn't ride much. :(
