If found please return to... or This book belongs to...

Hi,
I'm looking for a 4 x 6 card for an autograph book that is along the lines of "If found, please return to:" and then a space to fill info in. Or This book belongs to: Lexi Hill. If found, please call:" and then space to fill info in.

I thought I saw something like this in my tours through the many photobuckets here, but I can't find it now.

Can anyone point me in the right direction?
