I used a roll of masking tape. Each time we went out (this includes the drive up and back), I wrote his name, our names, our hotel and most importantly our cell phone # . I taped this piece of tape to the back of his shirt We had our cell phone on us at all times also. My ds is 3 and I told him if he didn't see us to ask a mommy to help him.
 
I've done the tape in the shirt. I also have done the note card in the pocket (as the girls got older). Most recently I purchased them guest of honor pins and put masking tape on the back with my cell phone number. I have told them that if we get seperated to tell someone who works there they are lost. If they can't find an employee then they are to find a "mommy" with children. I had read that they are the most likely to help a child.
 
We used a shrinky dink dog tag that we got at the pet store and pinned it to ds's shirt.
