I have a 4 day hopper pass from 1992....

....and it has 2 days left (back when it advertises that you can get into all THREE parks!).

I know that they don't expire, but my question is this:
Can I apply the remaining days to a 5 day hopper ticket? And if so can that be done at any Disney store or do I have to actually do that in the parks?

You would have to upgrade your ticket at WDW GS but personally, I would advise against that. The "upgrade value" is based on what you paid for your ticket originally and than prorated, Park admissions have gone up drastically since than and it would be costly for you. I would use those 2 days and than buy a 3 day hopper. It will be cheaper for you in the long run.

Would that prevent me from using fast pass? Do they take those tickets? or is fast pass only for your room card (will be staying onsite)?
 
I think the Fast Pass machine will take the old passes, but if not, there's always a CM there at the machines who could get you the fast passes by using their "card".
 

Do a no-cost exchange of the old pass for a modern magnetic stripe pass and the new pass will have the same two day's admission on it and you can get Fastpasses easily and quickly. Do this at Guest Relations when you get to WDW.

After using up the two days from the old pass, buy a new pass for the rest of your vacation. Again you can save unused days for a future trip.

Tip: Ask for the old pass back (which will be stamped Void) as a souvenir.

Tip: If at any time you happen to be carrying more than one pass in your purse or pocket (for example you bought a new pass before using up the old one) be extremely careful not to use the wrong pass when park hopping, you could consume two day's worth of passes on a single day which is very wasteful.

Tip: Nowadays, once old passes are modernized, they are good at Animal Kingdom too.

Tip: If you want "unlimited magic" (where your park ticket and room key are one and the same card) you probably do not want to involve or use your old pass at all. Your old pass will only buy about one day's worth of unlimited magic at today's prices so you are better off saving it for yet another future trip when you can get two whole days out of it.
