How to transfer ADR´s

What is the best procedure for transferring ADR´s from one person to another?

Can I just give someone my confirmation number and last name and can they use it or do you have to go through Disney dining?

i called and asked disney dining and they said just have the original holder of the the ADR to call and provide them with the new person's name and tel number.
