1) Don't know sorry - mine is part of the package
2) They charge $60 a bottle and it serves 4-6. The non-alcoholic is $24 a bottle (which is just silly because it is 1/2 the price retail)
3) I believe they start around $25 a person but that depends on your "add-ons". That doesn't include the location fee and the fireworks viewing fee. (This budget thread will have some breakdowns http://www.disboards.com/showthread.php?t=2182201)
