How do you get personalized menus? Does it cost anything extra?

I'm taking my Grandma to her favorite place, Citricos, on November 2nd for her birthday (she'd kill me if I said how old she's turning, haha). I really want this to be extra special. I already had them note the birthday when I made the ADR. I'd also like them to note her dietary restrictions. Where do I call for that and when should I make the call?

My BF and I were celebrating our anniversary and it was noted on the reservation that we were and they personalized the tops of our menus with our names...we did not have to do anything. They also gave us pins that said Happy Anniversary when we were there too...this was at Yachtsman Steakhouse
