Onboard, I wear a lanyard. In port, it's in my pocket.
The KTTW cards do not have a hole punched. And it's not really a good idea to punch one yourself after receiving it, as there's a chip in the card to open your stateroom door. No way of knowing exactly where in the card the chip is.
The lanyards that returning cruiser gets has a plastic, seal-able, pocket on it for holding cards.
I would get lanyards before you board, very limited in the shops and most are the pin trading lanyard / pouches. Never saw any for sale as nice and simple as the free ones the returning cruisers got (in the pic above) nor could we find one with a sealable pocket, and we spent entirely too much time looking onboard
I just slip it in my pocket. I successfully keep important things in my pocket the 51 other weeks of the year, so I see no reason to use a lanyard on a cruise.
The kids are different. I will use a lanyard for them.
Sometimes I wear the lanyard. Most times everything is in my pocket; or, if I don't have a pocket, it is in DH's pocket.
I have found some thin black lanyards w/ resealable pouches at Walmart. They are usually in the men's dept by the wallets. The last ones I bought were $0.99.
I wear the lanyard with pool clothes or cruise casual, but on dress-up nights I take the card pouch off the lanyard and put it on a badge reel. I clip it to my pants pocket or, if I'm wearing a dress, clip it to the neckline but hang the card inside my dress.
I use the lanyard too. When wearing nicer clothing, or often at dinner, I put it in a wristlet bag in which I also put my iPhone. I like the lanyard because I hardly ever have pockets (and I'm worried about dropping it out if I do), and I also use it carry my bean card from Cove Cafe and my excursion tickets when we're heading off to excursions. No worries about losing anything and hands are free. Also, with the ability to just tap your card to get on and off ship and to open your stateroom door, you just have to touch the lanyard to the reader - no need to dig into pockets.
