Help with a license

Hi, I am having a birthday party for my son around the cars 2 theme and I thought that it would be really cute if I could put a picture of each child into a drivers license from radiator springs. Does anyone have that? I can add the text (like name, picture, date and such) I just need a template to work from. I would love for it to have the radiator springs sign on top and a map of radiator springs where the words go (like a real license), but beggars can't be choosers and I will be happy with whatever anyone can come up with...
