The Disney official website's link to the Guidebook for Guests with Disabilities does list the attractions that have warnings for people with health conditions like heart conditions, high blood pressure, etc. so you can get that information there.
As Cheshire Figment posted, there is a link to the Guidebook in the disABILITIES FAQs thread.
Once you are at the parks, there is a little 'warning icon' triangle for the attractions with warnings both on the park maps and on signs at the entrance to attractions and signs before the boarding area. So, you will be able to avoid those attractions at the park, even without looking in the guidebook.
