In the past, I have been emailed a health form to print and fill out. However, this time I haven't received it yet. My cruise sets sail in about 2.5 weeks. Where can I find the form? I tried to pull up an old email but of course can't find the right key words to search for

I'd contact DCL, if you really want it to fill out beforehand. Or you can just wait and get one at the terminal.

They give you everything they can in the download section of the website. Under most circumstances, it is a fill out at the terminal type of form, because you know, your health can change in 2 weeks. Personally, don't worry about this one item and think more about the great fun you'll have on board!

We sailed B2B on the Disney Dream in December 2015 -- departing December 11 and 14.

DCL emailed us the health questionnaire form on November 30. The form is different from the form used when we sailed in 2014.

We sailed on the Fantasy two weeks ago and we were never e-mailed the health form. We had been e-mailed the form on our previous cruise in May. When we checked in at the terminal, they asked for the form and we told them we never received it. It wasn't a problem as they printed it out for us right there and we just filled it out. The families on either side of us also did not have the health form so I am not sure if they are still emailing it beforehand.

The last couple of cruises we took the health form was right with the online check-in info. We printed out the health form along with the signature form to take
with us to the terminal. Didn't see it for our upcoming cruise in May but I know they will have this form at the terminal check-in. Only takes a moment to fill
out. Bon voyage.

It's a very small, 15 second, 3 question form. If you don't print it out from Bishoparcs link (thanks!!), they have one at the port for you. Fast, trivial, easy, no worries. We've always done it at the port in seconds.

Heavens, you don't need to worry about not having it now, much less call your TA or contact Disney (!) for it.

I think they were sending out forms early last year because of a specific outbreak in a foreign country. The form is very short and can be filled out while waiting in line to check in.

My online check in doesn't have the health form as one of the options. Our last DCL cruise it was emailed to us.

On our Feb 2014, 2015, 2016 cruise we filled one out at the Port and this last one told we could have printed it online. When I recently booked for 2017 I asked where it was online. The CM didn't know but checked with the supervisor. She told us it was available on the old website but not on the new one. We were never mailed one in the past.

As I understand it, they are giving the health questionnaires out at port currently. We sail next Sunday and, although I have just yesterday had emails regarding the Zika virus issues, no health questionnaire. This leads me to believe that they are giving it out at port so they are sure of getting a response as at that date, since your health can change significantly sometimes in just a few days. Maybe there have been issues in the past where people have been filling the form in a couple of weeks before their sail date, and actually weren't in good health at the time of sailing. Who knows.

But, it makes sense. People saying "I feel fine" 2 weeks ahead of time really doesn't help on embarkation day.

That's what I was kinda thinking too. I think they're trying to encourage you to fill the form out the day of the cruise since the questions ask "In the last 24 hours have you.."

I had it emailed to me for our December cruise, but our cruise last week, I never got the email. They gave to us in check in.

Yep - they give it to you at check-in with a pen. It took me ~20 seconds to fill out. Don't sweat it!
