Just ask. Unless the ship is totally full and they absolutely CAN't make the change, they will. It is easier on the dining staff if you have your TA (or yourself if you booked thru DCL) make the request before you sail. That will take care of the dinner seatings. When you enter the dining room for breakfast and lunch, just tell them that you'd like to be seated alone.
As to has it been done...on our last cruise, we were two families who met prior to the cruise and asked to sit together...a total of 5 people. Of course, we were at a table for 6, but who cares about an extra chair?
