I have used CustomizedGirl.com many times in the past. They are fast, cheap, and you can usually find a discount code.

vistaprint.com has them for about $7. Look online for a code. I made them for my kids last year and we were very happy. I have used zazzle for other things in the past and we were happy with them as well. I find they are more expensive, but they ship faster.

I made 10 using CafePress a few months ago. IIRC, you could have ordered just one.

The quality of their shirts got better ratings than VistaPrint, which is why I went with them.
