Form for military

When I go to the seaworld website and click to view info on Operation Salute, it tells me I need a paper. It also tells me the form is available at the seaworld website, but that pop up is the only thing I can find! Does anyone know where on the website it is? It also says we can get a form at the park. Has anyone done that? Think it will be a problem waiting until then?

All you have to do is click on the operation salute box. It brings up a question and answer sheet and then click on the continue to go to the 2nd page. The form is there.
