The law requires that if the dividends are $10.00 or more they must send you a 1099-DIV. There is nothing which says that they cannot send you one of it is less than that amount. Some companies will send them to everyone. Even if you do not receive one, the law requires that you report them on your tax return.
There is the possibility that when you received your last dividend for the year that part of the advice notice was the 1099; you might want to look at that and see if it says somewhere on it that it is a subtitute 1099-DIV.
I do not believe Kodak issued dividends in 2009 because of how bad their results were, so if none were paid, regardless of the number of shares you own, you'll have no 1099-DIV.
