Form 1099 DIV

Nope. Most firms don't even send them if less than $10.
 
The mere fact that you received a 1099 DIV indicates that it was sent, to you, to the IRS. I would suggest reporting it.

In trying to find an answer to my question about the same form, I came across two different sites (one was Motley Fool) that said you SHOULD report the dividend no matter how small.
