Filling out a medical form/new patient paperwork

As part of my annual exams and due to family history, my OBGYN has me do a glucose tolerance test (GTT) from time to time.

Well, this time I apparently failed.

I made an appt to see an internist next week (who specializes in diabetes -- he was on the faculty at the med school here and has done extensive research with diabetes so that's why I chose him).

Anyway, I got the paperwork in the mail today to fill out and it asks the following:
Please check symptoms you have had related to the problem, and of course there's a long list.

Now should I give information about things I've noticed in the past week (i.e a low constant headache for the past four days) or should it be long term symptoms (like headaches for months which I have NOT had)?

It also asks about tiredness. How would I even begin to describe that? Depression -- I've recently come to the conclusion that maybe I am a bit depressed, but not down in dispair, lie in bed all day.....
 
Well since it's a recent diagnosis, some of your symptoms may only be recent. If you don't have a reason for it, like the headaches, I would check yes. But if you know there's another reason for the headaches, caffeine withdrawal or change in weather or sinus, then I might not check it, but let him know at the time of the appointment.

Depression, if it's just been a meh day or maybe week, I wouldn't check it. If you've had it before, and you're not at your lowest or heading to your lowest, then I wouldn't worry about it. It seems like what you have been dreading is finally creeping up on you and you're having to deal with it. I think that would put any one in a "meh" kind of mood, but not depression.
 
I would mark everything that you are experiencing, then when he is examining you and going over your paperwork, he can decide what is important at this time and what isn't.

I agree with this. Also, when your hormones are off it can mimic depression so you may not really be depressed, just "off". As you talk with the dr. he will have insight on the symptoms.
