Assuming that you stay in a standard room (not castle club), then you get one fastpass ticket (valid on any fastpass attraction) per person per day.
If you stay in a castle club room or suite, then you get an unlimited fastpass ticket, valid on the same set of attractions as many times as you like, for the entire duration of your stay.
The fastpass attractions are (I believe):
Disneyland® Park
* Indiana Joneset le Temple du Péril: backwards!
* Space Mountain
* Peter Pan's Flight
* Big Thunder Mountain
* Star Tours
* Buzz Lightyear's Laser Blast
Walt Disney Studios Park
* Flying Carpets over Agrabah
* Rock'n'Roller Coaster
* Tower of Terror
Anyone can use their entrance ticket to get fastpasses from the machines at the set intervals. These are special fastpasses that you don't have to use your entrance ticket for and so you can use both this fastpass and get others from the machines.

The FP that guests of the Disneyland Hotel use have a resctriction as to the times they can be used. Others who have used them will be able to confirm recent experiences
Yes - you can get your next fastpass either when the time on the first one is reached, or two hours after you collect the first one (depending on which happens first).

We've just come back (well a week ago) from staying at the DLH in a standard room and the FP was valid 2 - 6 pm each day.
Like others have said it's a good thing to have when it's busy as essentially you can hold 2 FPs at the same time - one from the hotel and one from the FP machines in the park. Tbh it does depend on the time of year though as I think we only used one day of our FP and we were there 4 days - it was v quiet so some FP queues weren't even working. But at Christmas, depending on the dates it could be good!
