What is the difference between EMH and MM? When you're planning 60 days out for character breakfasts, how do you know which park will have one of these on a specific day of the week?

DLR does not have EMH.

Early Entry is what guests staying onsite at a Disney property get at either DL or DCA each day.

MM is what people with a 3 day or greater ticket get once at DL.

Outside of peak weeks, EE is usually at DL on Tues, Thu, and Sat. Other days at DCA.

EMH is kind of the blanket term for both parks. Magic morning/ early entry is Disneyland; onsite guests get it every day it's available (Days with "T" in them) and 3 day or more parkhoppers purchased in advance come with 1 MM for Disneyland. (a few other tickets, too, depending on where you get them-- last time I looked, way.com had them on 1-park-per-day tickets of 2 or more days).

Early entry/ extra magic hour usually refers to California adventure, and only guests of the 3 Disney hotels get that. Sun/ Mon/ Wed/ Fri, normally.

If you're looking to book your dining reservation at one of the parks during MM/EMH (a good strategy if you don't qualify for the early entry), you can call the dine line to make your reservation and/or check the website. They can tell you the hours available for booking. If it's a MM day at Disneyland the breakfast at Minnie's will begin seating about 10 minutes after the park opens for MM. So 7:10 if it opens at 7:00. If you have a Magic Morning on your ticket you can still use this to your advantage. Example on Tuesday you use your MM at Disneyland then on Wednesday you have breakfast with Ariel and friends during DCA's EMH. Or Thursday breakfast with Minnie and friends.

Generally, as people mentioned above Disneyland offers MM to guests with most 3 day tickets once during the life of the ticket on either a Tuesday, Thursday, or Saturday. Hotel guests can also go to Disneyland for EMH on those same days and also to DCA on Monday, Wednesday, Friday, and Sunday. Non hotel guests do not get any kind of early entry to DCA (excluding dining).
