What is the difference between EMH and MM? When you're planning 60 days out for character breakfasts, how do you know which park will have one of these on a specific day of the week?

EMH is only available to onsite guests. If you are not staying at one of the three Disney hotels, you can't use it. If you are an onsite guest, you can use it every day you're staying onsite including check in and check out days. Meanwhile people with certain tickets will be given one day of MM, regardless of where they are staying.

EMH happens at both parks (usually alternating days), but MM only happens at DL. MM at DL happens on the same days as EMH at DL.

So basically, of you are staying onsite you can use early entry as much as you want at whichever park offers it that day. If you are stayong offsite but your ticket gives you MM, you can use early entry only once at DL on certain days.

I think the usualy schedule is DL EMH/MM on Tue, Thur, Sat and DCA EMH on Mon, Wed, Fri, Sun. This can change, but I think they only really change it during very busy times and special events.

EMH is like the blanket term for all early entry to DCA and DL. Only onsite hotel guests can use the early entry hour at California Adventure, which as the above poster said, is Mon, Wed, Fri and Sun.

MM is what they call it for just Disneyland. Onsite hotel guests can always get in an hour early on MM days. But, if you bought tickets ahead of time that include a MM, you can get in early one of the MM days, Tues, Thurs, Sat.

Character breakfasts are always the same regardless of when/what days there is EMH.

There are only two character breakfasts inside the parks: Plaza Inn (Minnie and Friends) and Ariel's Grotto. At this point, it seems like Ariel's doesn't change the time it opens based on EMH. Plaza Inn, on the other hand, seems to have its first ADR 10 minutes after the start of EMH/MM or 20 minutes before the park opens on non-EMH/non-MM days.
