Experience/Designs for photo thank yous (non Disney?)

Hello! I was wondering if any of the wonderful and creative disigners on this site have experience with photo thank you cards? I am going to try to explain the idea I have and hope it makes sense, lol!!

We had a Thomas birthday party for my son and I'd like to send out thank you note "pictures," size 4x6. I am imagining a Thomas background with a square "cutout" for me to insert a picture digitally, so for example, the entire left side of the card would be a picture of Thomas and the right side would say "thank you for celebrating my 3rd bithday with me!" Love, Patrick with a picture of him.

Is this even possible? Where would I begin? Does anyone have a Thomas disign that would accommodate this?
