ESTA - Different Surname, 1 form or 2

Sorry if this has been asked before but going to Orlando 11th August and just about to complete my ESTA from online. Kept my maiden name on passport and therefore have different surname to husband and son. Do I need to complete 1 form or 2? Pretty sure I only completed 1 when it was still paper based.

Everyone has to fill in their own ESTA, you need one per person.:thumbsup2
 
Thanks. Think I was confused. Did there use to be 2 different forms that we got on the plane. An individual one and then 1 for entire family?
 
The customs form is 1 per family, the one where you declare anything you are taking, food or over $10,000 etc.

I did the same as you and kept my maiden name and just filled out forms for both DH and myself. Will be changing when I send off for my new passport next year though.
