Emergency Shirt Help Please!

We have made shirts before, but have never had this many problems...the tranfer is not coming off cleanly and leaving the shirt looking spotty or flaky. What transfers do you use? I know that Amy's are good, but I do not have time to have them shipped to me. Suggestions anyone?

I used this for our shirts from last summer.
Everyone has been washed several times.
DS (age 2.5) has worn his weekly since our June trip and the iron on is still secure - no cracking.

I think I bought them at Staples. Whatever brand you buy - make sure it is DARK t-shirt transfers.
