We're much farther aft than we like to be (booked a GTY and just got our assignment--not a VGT, as those cant be changed). We aren't necessarily looking for an upgrade, just a comparable stateroom farther forward. Our PAT is 12:30. What time should we arrive at the port to begin attempting check-in, in order to see what rooms are available to switch to.

And I guess also regarding upgrades... we booked a 5C FLR rate, and were placed in a 5A. Will the upgrade offers (if any) be based on the fare paid (5C FLR) or the existing room (5A)? I assume it would be cheaper to go from a 5A to a 4x than it would be to go from a discounted 5C to a 4x. Just want to know what to expect.

The cost difference is based on the price paid, not the price of the room you were assigned to. You'll be going from a 5C to a 4x.

With a 12:30 Port Arrival Time, you may be asked to wait for the terminal to clear (especially for a Dream/Fantasy cruise). PATs were set up to spread out the crowds and the Dream class ships hold more people than the terminal was originally designed for.

Once you are allowed to enter, you can check in, but your boarding number will be the same as if you arrive at your correct PAT.

If you don't mind waiting in line, you can arrive anytime after 10:30 (when terminal opens), however boarding doesn't begin until 11:15-11:30, and that's when the terminal starts to clear out a bit, so you may not be allowed in right away.

Also, since you're only looking for a lateral change, you may want to give others (who may upgrade out of those rooms) a chance to do so.

has anyone ever added a room as an upgrade We are 4 adults in a room wondering if adding another room would be possible and the est cost

I would seriously doubt that you could add a room once at the port. It is theoretically possible (because there may be empty rooms), but I would think not so easy. It could be that all the lifeboat stations are maxed out, even if there were available rooms. Or that DCL likes to have a small percentage of rooms vacant for possible issues resolved by moving people around.
