This is our first DW trip and I'm not sure I completely understand the dining plan so any help is appreciated.

Are we able to order anything off the menu or are we restricted to certain food items?

I have seen others mention that shakes are considered a drink at some restaurants, how do I know which?

Yes. You can order anything off the menu's. However, I think there are a couple of dishes at some restaurants that do have an additional fee, but I am not 100% sure what dishes, which restaurants or if I just made that up. It sounds good though.

Shakes at places like 50's PTC and Sci-Fi can count as either a drink or dessert. And being on the dining plan you are entitled to both. What my girlfriend and I have done is order a normal beverage with our meals and then split a shake and a dessert (a shake and 'smores at 50's is a great ending to a meal). We used shakes as our beverages, once, and it was just way too much for us for one meal.
 
We were on the Delux Dining Plan in December 2008 and really enjoyed it although it was a LOT of food. We didn't experience many things you couldn't have and those that were in that category you could order for an additional surcharge. We found the servers very helpful at explaining what was offered and what was not, so if you're unsure just ask. Paying OOP for some items can still save you money. And for us the attraction was the fact that it made the vacation more hassle free (food was essentially paid for upfront so we did't have the budget worries we would normally have) and we could have whatever we liked when ever we liked it. We do wish that there was something between the Delux plan and the ordinary dining plan, something perhaps with 2 TS and 1 CS per day or that nature, but all in all, we found it worth our while.

We did Teppan Edo, Crystal Palace, Playhouse Disney breakfast, Safari Breakfast, CRT, Mickey's Backyard BBQ, HDDR, Tutto Italia, Mama Melrose's and Whispering Canyon Cafe as well as the Pop food court and a few QS places. Since I dislocated my knee at Crystal Palace on day 2 and then proceeded to come down with a severe sinus infection and fever (:sad2:), we did cancel a few ADRs as well, but all in all we found it good value and easy to use.

For the most part, you are able to order anything on the menu.
We've done the deluxe plan twice and are doing it again in November. Your server will explain what is in included (which is mostly everything if not everything at most places, there are a few exceptions but I've been to a LOT of TS and there aren't many and I can't even give an example/remember other than at Cali Grill some of the sushi is an entree and not an app).
With the deluxe you get an app (for lunch & dinner) - this is a salad (non entree), soup or a cold/hot app (as long as it doesn't say for 2) - if it does say for 2 then 2 people have to have that as there app and share - there are a few apps out there like that.
You get an entree from the entree or sandwich or pasta list.
& then a dessert.
For drinks - you get 2 for breakfast- juice and coffee (or other beverage) & 1 for lunch & dinner -soda, tea, coffee. As for shakes - your waiter will let you know if you can include it - but at 50s Prime Time & Le Cellier I know the special drinks are included - just ask if you have a question or they usually go through the menu anyway.
